If you have previously been taking GLP-1 medications such as Mounjaro®, Wegovy®, Ozempic®, or similar treatments, we require evidence of your prior treatment before we can continue your prescription. This helps our clinical team understand your treatment history and ensure safe, appropriate care.
What Evidence We Accept
We accept several types of documentation as proof of your previous GLP-1 treatment. All evidence must clearly show four key pieces of information:
Your name (patient name)
Medicine name (e.g., Mounjaro, Wegovy, Ozempic)
Dose strength (e.g., 2.5mg, 1mg)
Date (prescription date or dispensing date)

Official Dispensing Label (Preferred)
The pharmacy label attached to your medication box or bottle.
Requirements:
Must show your full name as it appears on your prescription
Must display the exact medicine name and dose
Must include the dispensing date
Label should be clear and legible
Copy of Prescription
The original prescription issued by your GP or private prescriber.
Requirements:
Must be an official prescription form
Should include prescriber details and signature
Must show your name, medicine, dose, and date
Can be from NHS GP or private healthcare provider
Pharmacy Dispensing Record
Official record from your pharmacy showing your medication history.
Requirements:
Must be from a registered pharmacy
Should show your dispensing history for GLP-1 medications
Must include your name, medicine details, dose, and dates
Can be printed record or official pharmacy documentation
