To complete your clinical assessment and ensure your treatment is safe and appropriate, we require verifiable evidence of your current weight.

Accurate and up-to-date weight information is essential for safe prescribing and dosage decisions. This verification step is a mandatory part of our clinical governance and safety protocols for online pharmacies, ensuring your treatment is prescribed in line with UK clinical standards.

You can verify your weight using one of the three methods below.

Method 1: NHS App Screenshot

You can provide a screenshot from your NHS App showing your most recently recorded weight.

Requirements for this method:

  • The weight must have been recorded within the last 30 days.

  • Your full name must be clearly visible in the screenshot alongside the weight reading.

Method 2: Local Pharmacy Verification

You can visit a local pharmacy and ask them to weigh you and provide written verification.

Requirements for this method:

  • The verification document must be dated and stamped by the pharmacy.

  • The date must be within the last 30 days.

  • Your full name must be clearly visible on the document.

Method 3: Photo on Your Scales

You can take a photograph of yourself standing on your weighing scales at home.

Requirements for this method:

  • You must be in full view (head to foot) stepping on the scales.

  • The weight reading on the digital or analogue display must be clearly visible in the photograph.

How to Submit Your Evidence

Once you have your screenshot, document, or photograph ready, you can upload it securely through your Bolt Hub during your check-in or enrolment process.

If you have any difficulties obtaining this evidence, please contact our clinical support team for guidance or simply email us at [email protected]
